Medical Form
All full-time students must have a physical exam no less than 12 months prior to enrolling at Emmaus and Student Athletes must have a physical exam each year prior to participation in athletic practices or contests and within one year of all athletic sponsored practices and contests. We have a Physical Examination & Medical History form that needs to be completed. The physical examination portion on page 1 must be completed by your healthcare provider and the medical history portion on page 2 should be completed by you.

Student Health Plans
All full-time resident students are required to have a health plan, which defrays the cost of hospitalization and medical-surgical coverage in the event of sickness or accident. If you are covered by a health plan, you must provide Emmaus with evidence of adequate coverage and sign a Statement of Coverage form. If you are NOT covered by your own (or family) plan, then you must purchase the Fixed Indemnity Plan for injury or sickness provided through the college.

Housing Plans
Most students who attend Emmaus Bible College are required to live on-campus. There are some exceptions to this requirement including married students, students living with a parent, grandparent or legal guardian, students who are 24 years old or older, or part-time students. Submit your on-campus housing preferences here.
Meningococcal Vaccine Information
You are not required to receive the meningococcal vaccine to be enrolled at Emmaus Bible College, however, we are required by law to inform you about the vaccination and keep track of the number of students who have received this vaccination.
